From the moment you are referred to us, we will appoint a Housing Liaison Officer who will support you throughout the entire process. They will:

  • Attend all multi-disciplinary team (MDT) meetings with commissioners, support providers, and social workers
  • Act as your advocate in all housing matters
  • Guide you through your transition into your new home

Preparing Your Home

We make sure your property is:

  • Safe, secure, and ready for you on your agreed moving day
  • Adapted to meet your needs, including supporting applications for grants and funding to make necessary alterations

Financial Guidance

We will help you:

  • Apply for the correct benefits to cover your rent
  • Understand the costs involved and how everything works

Ongoing Support

Once you've moved in, we will continue to support you by:

  • Providing guides and information on looking after your home
  • Helping you (or your support staff) report issues and maintenance needs
  • Visiting regularly to make sure your home is being well maintained
  • Setting up and managing utilities on your behalf
